You must register or login with a Facebook ID to take the survey. The information will be only used to contact the winner of the iPod touch.

If you choose to register and not use a Facebook ID to log in, a password must be provided to keep your personal information secure.

After registering or loging in with your Facebook ID, click on the 'Take the Survey' tab to take the ADDA Vitality Survey. All surveys will be anonymous.
